Withdrawal Form

In order to discontinue any lesson with AFS, it is studio policy that a fifteen (15) day written notice must be given to the office. Any notice received after the 15th of the month will be considered dated as to the 1st of the following month. Students are allowed and encouraged to attend their classes until the end of the month of their last tuition payment, as there are no refunds on tuition. Please note that costume payments made for dance students before November 20 will be refunded. After November 20, no costume refunds will be given.
